- The names, addresses, phone, and license numbers of all drivers and passengers involved in the accident.
- License plate numbers of all vehicles involved in the accident.
- The make and model of each car.
- Insurance identification and the agents' contact information.
- The names, addresses, and phone numbers of any witnesses.
- The names and badge numbers of the police officers.
